fix multiple DEFAULT NULLs

    $info->{default_value} = \'null'

is unsafe because Data::Dump optimizes
refs to constants, so we must make sure to use:

    my $null = 'null';
    $info->{default_value} = \$null;

for all scalar refs in the dump.
