Documentation change: do not overwrite unchanged schema modules

This patch only touches documentation. It adds a mention that unchanged
schema modules will not be overwritten. An entry is also made in the
Changelog.

Prevent Schema::Loader from overwriting unchanged schema files

previously, the make_schema_at feature of Schema::Loader would overwrite
files in the dump_to_dir regardless of whether the underlying database
schema had changed or not. This patch causes Schema::Loader to check
the contents of the current file with the newly created schema from the
database. The file is only overwritten if the two differ. This means
that the old timestamp and creation date are preserved.

Fix Oracle constraint detection for non-owned schemas

Bump Module::Install requirement to make sure we have both test_requires and auto_install

Remove and ignore autogenerated files

Remove Build.PL, no longer supported by Module::Install

Version bump for release

Don't execute the column_info sth, it's already active.

Fix DB2 support:
 - foreign_key_info needs the PK schema name
 - up/down-case table names when going to/from the DB

Make sure to get the constraint columns in the right order.
