Integrate perlio:

[ 16216]
Fix weird warnings and/pr segfaults on binmode(,"encoding(...)")
- if encoding loads Encode then stack grows.
- pp_binmode was not allowing for that to happen
- added PUTBACK/SPAGAIN.
